To apply for a veterinary bill assistance grant, please complete the following steps:

1. Complete the application form below

2. Email your supporting documentation to: info@theguinnessdunnfoundation.org

   • Estimate from your veterinarian

   • Proof of employment income (two months of paystubs)

OR

2. • Proof of benefit income (ex. Social Security, Disability, Food Stamps, etc)

PLEASE NOTE: applications received without the required documentation will not be presented to the board for review. NO EXCEPTIONS.
